All About Spinal Disc Herniation

Herniated Disc is one of the most common causes of back pain! Do you know where you back pain is coming from?

The bones (vertebrae) that form the spine in the back are cushioned by discs. These discs are round, like small pillows, with a tough, outer layer (annulus) that surrounds the nucleus. Located between each of the vertebra in the spinal column, discs act as shock absorbers for the spinal bones.

A herniated disc (also called bulged, slipped or ruptured) is a fragment of the disc nucleus that is pushed out of the annulus, into the spinal canal through a tear or rupture in the annulus. Discs that become herniated usually are in an early stage of degeneration. The spinal canal has limited space, which is inadequate for the spinal nerve and the displaced herniated disc fragment. Due to this displacement, the disc presses on spinal nerves, often producing pain, which may be severe.

Herniated discs can occur in any part of the spine. Herniated discs are more common in the lower back (lumbar spine), but also occur in the neck (cervical spine). Common Causes – A single excessive strain or injury may cause a herniated disc. However, disc material degenerates naturally as one ages, and the ligaments that hold it in place begin to weaken. As this degeneration progresses, a relatively minor strain or twisting movement can cause a disc to rupture.

Certain individuals may be more vulnerable to disc problems and, as a result, may suffer herniated discs in several places along the spine. Research has shown that a predisposition for herniated discs may exist in families with several members affected.

Kirti Saini is a compassionate and dedicated physical therapist with over two years of professional experience. She holds a Bachelor’s degree in Physical Therapy and is currently pursuing her Master’s in Exercise Science and Kinesiology from Long Island University.

Yash Upalekar is a passionate physical therapist with a Master’s in Exercise Science and a commitment to bridging the gap between injury and performance. Hardik Parikh, PT, DPT, MPT, COMT

With over 15 years of experience, Dr. Hardik Parikh brings a deep blend of clinical expertise, academic rigor, and patient-centered care to his practice. Fluent in English, Hindi, and Gujarati, he has worked extensively across neurological, musculoskeletal, and cardiorespiratory rehabilitation in both hospital and home-care settings.

He holds a Doctor of Physical Therapy from the National University of Medical Sciences (USA), a Master of Physiotherapy in Neurological Disorders from Parul Institute of Physiotherapy (Gujarat University), and a Bachelor of Physiotherapy from K.J. Pandya College of Physiotherapy. In addition, he earned his Certificate in Orthopedic Manual Therapy from Capri Institute in 2017.

Mehak Goel is a dedicated physiotherapist with a Master’s in Exercise Science from Adelphi University, New York, and a strong foundation in evidence-based practice. Fluent in English, Hindi, and Urdu, she brings empathy and precision to every session, helping patients regain movement, confidence, and quality of life.

Her expertise spans musculoskeletal and neurological rehabilitation, with additional training in taping, cupping, myofascial release (MFR), and instrument-assisted soft tissue mobilization (IASTM). Foram Bhatt brings over six years of hands-on clinical experience and a deep commitment to patient-centered care. She is fluent in English, Hindi, and Gujarati, which helps her connect effortlessly with patients from diverse backgrounds. Foram’s therapeutic approach blends advanced manual techniques with an intuitive understanding of movement and recovery.

Her certifications in Advanced Manipulative Therapy and Mulligan Mobilization strengthen her ability to restore mobility and function with precision and care. Ms Granda (Jackie) is a binding force for our younger staff members. Ms Granda is very graceful in her interactions with patients and colleagues alike and therefore she is well liked by all. Ms Granda genuinely believes Dynamic to be her second family. Outside work, Ms Granda enjoys walking and reading and she spends most of her time caring for her beloved family.

Ms Taslima Rahman grew up in Bangladesh and has a great command over connecting with people. She has been a part of our Dynamic team since the past 1 ½ years and is a licensed Physical Therapist Assistant who brings a smile to every patient’s face. Shweta Miglani

Ms Shweta Miglani has clinical experience of 13 years and has been a part of our team at Dynamic since the past 2 years. Shweta has vast experience in outpatient Physical Therapy and has successfully run a private practice in India before moving to NY. She is a very caring clinician who loves talking – she can go on listening and talking to her patients all day long and deliver compassionate one on one care. She is a mother of a young boy but has energy to pursue her doctorate in Physical Therapy from University of Montana. Outside her work, Shweta loves reading, playing word games and sketching.

Mitsu is a master clinician who has been striving daily at Dynamic Physical Therapy since the past 5 plus years to leave a positive lasting impression on every patient she meets. She prides herself in working one on one with her patients and is known to develop meaningful and compassionate professional relationships. She has received a Bachelor’s degree in Physical Therapy ( S.P university, India) followed by Masters in exercise kinesiology ( Long Island university , Brooklyn Campus ). Additionally, Ms Purohit is also a Certified Performance enhancement specialist (PES) & corrective exercise specialist ( CES) from National academy of sports medicine (NASM). Rajeshwari Soni grew up in Mumbai, India. She completed her Bachelor’s in Physical Therapy from Pravara Medical Institute, India in 2007. She pursued her Master’s Degree in Kinesiology with a major in Biomechanics- clinical track in 2011 from Texas Woman University, Denton Texas.

She has been working as a licensed Physical Therapist in the state of New York since 2011 and during practice she realized that timely and accurate diagnosis helps in faster recovery of patients and improves functional prognosis. Rajeshwari has been an integral part of our team since the last 5 years. Since 2016, she is appropriately utilizing her diagnostic skills to help patients’ achieve their goals. She has an RMSK- Registered in Musculoskeletal Sonography from APCA (Alliance for Physician certification in advancement) certification and also practices EMG/NCV testing, of which she received certifications from HODS (Hands On Diagnostics). Outside of her work she enjoys time with her family, loves reading, fitness/working out and helping people.

Dr Kajal Jani grew up in Gujarat, India. She has clinical experience of 20 years and has served our community in New York in a variety of settings such as in-patient rehab, group home and outpatient rehab. Dr Jani has been a vital part of our Dynamic family since past 10 years and has mentored numerous staff members. Her compassion and work ethics have earned her a lot of respect from her patients and colleagues alike.Since 2016, she is appropriately utilizing her diagnostic skills to help patients’ achieve their goals. She has an RMSK- Registered in Musculoskeletal Sonographic from APCA (Alliance for Physician certification in advancement) certification and also practices EMG/NCV testing, of which she received certifications from HODS (Hands On Diagnostics). Outside of her work she enjoys time with her family, cooking and yes: kickboxing.

Dr. Esha Desai, PT, DPT, MPH

DOCTOR OF PHYSICAL THERAPY SPECIALIST IN VESTIBULAR AND GERIATRIC REHABILITATION

With over fifteen years of clinical experience, Dr. Esha Desai has been an integral part of the Dynamic Physical Therapy team for more than a decade. She holds a Doctorate in Physical Therapy from The College of Saint Scholastica and a Master’s in Health Administration from Fairleigh Dickinson University.

Fluent in English, Gujarati, and Hindi, Dr. Desai specializes in Vestibular Rehabilitation, helping patients overcome vertigo, dizziness, and balance-related challenges. Her expertise also spans orthopedics, postural restoration, and geriatric gait and balance therapy. She believes that true healing begins with understanding a patient’s story, setting clear goals, and creating a collaborative path toward recovery.

Dr. Desai’s approach combines evidence-based care, compassionate guidance, and functional movement training to restore mobility, confidence, and independence.

Outside of work, she enjoys spending time with her daughter, reading, cooking, traveling, and discovering new cafes.

Fun fact: Her love for books began at the age of five, and she still treasures the first one that sparked her lifelong curiosity and passion for learning.
